In this role, you will serve as a trusted advocate and liaison, seamlessly coordinating services with various agencies and educational institutions to provide a comprehensive support system for our youth. Your ability to effectively communicate and collaborate will be crucial in ensuring their educational, therapeutic, and aftercare needs are addressed holistically.

Key Responsibilities:
Coordinate resident placement needs and maintain effective communication with all involved parties
Complete required intake paperwork for youth within the specified timeline
Maintain accurate and timely records, and bill indirect client contacts weekly
Collaborate with the clinical team to coordinate school and vocational matters for residents
Schedule review meetings for all youth, complete review paperwork, and obtain necessary signatures
Transport and participate in youth court appointments and evaluations
Arrange and schedule youth visits and passes, communicating with relevant parties
Coordinate discharge planning and make appropriate referrals for support services
Attend meetings as requested, participate in team meetings, provide crisis management services, and obtain required staff training hours
Education and/or Experience Qualifications:
Bachelor's degree in Human Services or a related field is required
Experience working with children and adolescents is preferred
Possession of a valid driver's license and the ability to obtain a Chauffeurs license (Class E)
Required License/Certification (one of the following):
- Chauffeurs license (Class E)
