Reporting to : VP of Enrollment and Growth Strategy

Job Summary: The Enrollment Advisor works with the incoming digital referrals and calls to help facilitate

setting up services for new and existing clients to receive

homecare. He/she is responsible for processing daily referrals received from digital marketing

sources such as online campaigns, web site requests, incoming calls

with eligibility and guide them through all service options to receive the best care based on

their needs.

Job Responsibilities:

  • Consistently maintain a caseload of home care referrals and manage the process of

converting referrals into an active home care case

  • Conduct immediate follow up with online requests, inquires and live chatter

  • Serve as the liaison between the contracts, Family and clients to get them eligible for services

  • Manage and initiate the intake process to help setting up services for new and

existing clients to receive Homecare

  • Schedule State and MLTC nursing assessments as needed to assess patients to

determine home care virtually or in their homes

  • Verify patient’s insurance to determine eligibility through E-Pace system

  • Investigate type and level of insurance coverage to assess patient eligibility for home

care services

  • Assist with Medicaid application to ensure the eligibility for home care services

  • Educate patient on True care offered services, screen patients to find out the best level

of care; obtain all required paperwork

  • Gather patient documentation and obtain any information needed from the patient to

get them cleared for services

  • Ensure referral forms are completed correctly for submission to contract, and

assisting with securing any supporting documentation needed

  • Complete data entry in and all communication notes to ensure accuracy of reporting and

outcomes of scheduled assessments and reassessments of patients and using Salesforce

  • Maintain timely communication with the Intake Manager, team members

  • Maintain effective relations with all agency contacts

  • Manage the referral intake process from initiation through admission statuses

    Job Requirements:

  • High School diploma from an accredited school/ GED equivalent

  • Experience working with ePaces and Salesforce is a plus

  • Has a sense of urgency in getting the job done

  • Personable and professional

  • Knowledgeable of CDPAP/ MLTC programs

  • Experienced with Intake, or knowledge of the HHA Exchange a plus

  • Able to work in a team-building environment

  • Bilingual (Spanish)
